Will facilitate coordination and development of a comprehensive and individual plan of care in collaboration with patient, family, PCP and multidisciplinary team.

 Required Qualifications:

• 2+ yrs experience as a provider in hospital setting
• ATLS, BLS, ACLS, PALS
