Job Description:

The Chronic Disease Coordinator will be responsible for coordinating the chronic disease program, which involves planning, organizing, directing and leading the development of chronic disease program protocols and standards of care.

Required Qualifications: 

  • BSN
  • Active RN license in the state of Nebraska
  • 3+ yrs experience working in a Community Health Facility
  • 1+ yr experience working with Chronic Disease Management
