Hospice Nurse Case Manager-Colorado

Job Description:

Monday-Friday 8am-5pm with rotating on-call. Case Manager is responsible for services to patients assigned to their case load. Will be responsible for ensuring patient’s medical, psychosocial, resource and spiritual needs are identified and a plan is developed and implemented to address identified needs.
