Physical Therapist Case Manager – Iowa

Day shift, no weekends, no holidays
PT Case Manager Job Description:
This is a full time PT Case Manager (direct patient care) provider. The position is responsible, as well, for PTA supervision. The health care environment is Certified Home Care. Experience in home care is preferred, however not a requirement. Our therapists see 4-7 patients per day and are weighted by need.
Never a shortage of work. Therapy team meets one time per week to discuss pts and participate on team IDTs. We have some insurance contracts so do some work with younger folks and lots of debility, decondition type work too. We do not go into facilities, except those that are working with us on the continuum of care.
PT Job Requirements:
- 2+ yrs experience
- Licensed in the state of Iowa

Able to recognize problems, identify the cause (technical or instrumental) and make simple corrections by using present strategies.
