Position Summary:
We are looking for a motivated, enthusiastic and skilled Physical Therapist to join our team! Your day will consist of evaluating and treating patients under the orders from the referring physicians. You will work in one of our state of the art facilities in a collaborative environment with other therapists. We have a therapist to aide ratio that allows our therapists to have more one on one time with their patients. This specific position is for our busy outpatient clinic that predominantly treats common orthopedic injuries but also serves neuro and pediatric populations.

Job Description:
Provides an efficient and effective process and structure that supports the acceptance and bed assignment of outpatient surgical;/procedural, observation and admitted patients from all portals of entry within defined performance expectations. Develops and achieves specific annual organizational and departmental goals established relative to clinical efficiency, effectiveness and throughput. Assures compliance with the regulations of all Federal, State, local and other regulatory bodies. Continuously monitors the outcomes of hospital-based clinical care activities to identify opportunities for improvement.
Supervisory Responsibility:
The Director of Care Coordination position requires direct responsibility for the management of multiple departments with large numbers of staff assigned to each. The incumbent directly and indirectly supervises multiple directors, managers, assistant managers, and/or supervisors.

MDS COORDINATOR- NEVADA
PERMANENT FULL TIME DAY SHIFT
JOB DESCRIPTION:
The MDS Coordinator is responsible to assess and/or coordinate the assessment of all residents and to complete the Resident Assessment Instrument (RAI), the electronic transmission of RAI data to the State of Nevada, the transfer of Resident Assessment Group Data (RUG) to the hospital PatCom system, and the ongoing education of those completing the MDS. Demonstrates the knowledge to complete the RAI and aforementioned responsibilities. Facilitates communication between the Interdisciplinary Team.
REQUIREMENTS OF MDS COORDINATOR:
- Graduation from an accredited School of Nursing.
- Current licensure by the Nevada State Board of Nursing as a RN.
- Certification in geriatrics or MDS desirable.
- Experience with RAI
- ACLS/BLS certification
- 5+ yrs of experience with at least 2 yrs in Long Term Care
