- Graduation from an accredited School of Nursing is required.
- Certification Required, BLS
- Valid State of Washington RN license required.
- Compensation: BOE
JOB SUMMARY The Director of Care Management has primary responsibility for the systems and staff that support patient transitions of care to ensure safe discharge, post-acute care placement and continuity handoffs to ambulatory care setting. The Director is a resource to the Facility Care Team in the development of and effective use of population management methodology, care coordination strategies, intra-disciplinary coordination of care, complex case coordination, patient education/involvement methods, and effective discharge planning. The Director ensures the maintenance of care coordination records to meet the needs of the organization and regulatory bodies. Key to this position is the collaboration with colleagues and peers. |
