Clinic Nurse Facilitator – Colorado

Job Description
Utilize professional nursing theory and the nursing process to implement, coordinate, and direct patient care and auxiliary nursing care activities, using a community health center primary care model. The Clinic Nurse Facilitator will also train new staff how to consistently deliver health care related services according company policy and procedure and nursing leadership best practice and perform other related duties as required
Required Qualifications:
- BSN
- Current Active Colorado RN License
- 2+ yrs experience in primary outpatient care
